Are you passionate about providing compassionate support and ensuring a seamless client experience? Join our dedicated team as an Intake Coordinator, where you'll play a key role in welcoming new clients and guiding them through the intake process with care, efficiency, and professionalism.
As our Intake Coordinator, you'll be the first point of contact for potential clients. Your mission? To ensure every inquiry receives timely, empathetic, and effective support. From gathering essential information to coordinating initial care visits, you'll help clients feel supported and understood from day one.
Serve as the primary contact for incoming client requests and guide them through intake documentation.
Collect and maintain confidential client information with accuracy and discretion.
Collaborate with our care team and referral sources to coordinate initial home care visits.
Track inquiries, resolve issues, and document outcomes in an organized system.
Maintain a well-organized database of client records, referrals, and communications.
Generate and analyze reports related to client satisfaction and referral metrics.
Work cross-functionally with internal departments and external partners to ensure seamless care delivery.
Identify and suggest opportunities for improving processes and client experience.
High school diploma required; associate’s degree or higher preferred.
Minimum 2 years of experience in intake, customer service, or a similar role.
Exceptional communication skills—written, verbal, and interpersonal.
Highly organized with strong attention to detail and time-management skills.
Comfortable navigating client management or scheduling software (e.g., Wellsky, Ersp).
Compassionate, empathetic, and calm under pressure.
Flexibility to work evenings or weekends as needed.
Love creating positive first impressions and building trust with clients.
Are a natural problem solver who stays organized under pressure.

Work Environment:
Office-based role involving extended periods of computer and phone use, occasional walking and lifting (up to 50 lbs).
