DEFINITION:
The RN Care Advisor is a registered nurse who provides clinical oversight to clients in their residences (private home or facility).
LINE OF AUTHORITY:
Reports to the President/CEO for clinical issues and to the General Manager for day-to-day operations.
QUALIFICATIONS:
- Graduate from an accredited school of nursing and currently licensed as a registered nurse in the state of Florida.
- Must have physical abilities to adequately perform described duties.
- Ability to learn and apply professional training, follow physician’s orders and produce accurate records of activities and services provided.
- Able to see and hear adequately to respond to auditory and visual requests which relate to the coordination of Agency activities.
- Able to speak in a clear, concise voice to communicate direction for Agency activities and staff.
- Demonstrates adequate mental acuity to perform duties stated in job description.
- Ability to organize work procedures, assume responsibility and effectively supervise others.
- Demonstrates problem solving abilities; positive interpersonal communication skills are essential.
- Demonstrates adequate knowledge regarding exposure to body fluids, and chemicals, and some hazardous equipment.
- Demonstrates exceptional assessment skills.
- Demonstrates ability to effectively communicate with patients, physicians, and staff.
RESPONSIBILITIES AND DUTIES:
- May represent the Agency on clinical matters as they relate to home care.
- Verifies the skill competency levels of all hired home health aides and CNAs.
- Serves as a clinical advisor for matters regarding clinical oversight.
- Makes initial evaluation visits.
- Re-evaluates client care needs on a regular basis.
- Participates in in-service programs, trains, and teaches other personnel.
- Develops Plan of Care with client and/or family members to meet the care needs of the client.
- Initiates preventative safety procedures as appropriate for the clients.
- Counsels the client and family in meeting care needs.
- Maintains confidentiality of patient and agency matters.
- Prepares written instructions for care provided by home health aides.
- Performs additional duties as assigned by the President/CEO or the General Manager.
- Enters client information including assessment and Plan of Care (POC) into Wellsky. Reviews caregiver daily notes and enters notes in ACT and revises POC as needed.
- Shares in the on-call rotation.
- Establishes initial client schedule in Wellsky.
- Assists in scheduling when needed.
- Works closely with the Scheduler, other Care Advisors, the Health Services Coordinator and the caregivers when assignments are being made.
- Submits required documentation in a timely manner.
