LPN - Position Description
Administrative Responsibilities:
- Monitor and report changes in patient symptoms or behavior including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points and reports for each shift
- Collaborating with RN Administrator, physicians, or specialists regarding patient care
- Evaluate the patient and home situation to determine what health teaching will be required, if additional services are needed and the frequency of this service
- Document services and education provided to the patient, using an Electronic Medical Record (EMR) application, and patient scheduling. MUST be able to adhere to strict deadlines for Start of Care (SOC), revisits, re-certifications, and discharges
- Participate in Case Conferences, In-Services and mandatory Staff Meetings as needed
Patient Care Services Performed:
- Take patient vital signs as required
- Suction via trach per the patient's plan of care
- Tube feed administration, medication administration
- Ventilator Maintenance, and pulse ox readings as recommended
- COPD- Congestive Heart Failure (CHF)-With RN Administrators guidance: Monitor and provide education on daily weight logs, fluid restriction education, medication management, vital sign teaching and reporting. Oxygen therapy management with energy conservation plans of care.
- Diabetes- diagnosis's education and compliance management, medication management, Physicians report, pharmacy refill fill management in coordination with RN Administrator
- Hypertension management (HTN)- Vital sign monitoring and reporting, In home PT/INR checks, Medication education, medication refill Management, physicians reporting
- Colostomy Care
- Recent hospitalization or surgery
- Wound Care and infection monitoring
- Chronic Urinary Tract infection in home management with in UA testing, and physician reporting.
Qualifications:
- One year of experience with home health and clinical knowledge
- RN or LPN from an accredited school
- Ability to act swiftly in an emergency.
- Flexibility and patience to work in a variety of living environments.
- Must have a valid, unencumbered License Practical Nurse (LPN) from the State of Michigan
- Must have a minimum of 1 year of Skilled Home Care / Visit nursing experience
- Must have a valid Driver’s License, your own reliable transportation and proof of current Auto Insurance.
- Must be willing to travel within a designated radius (from your home address); this radius will be agreed upon during the interview process and upon hire
