Are you a seasoned social worker with a heart for helping older adults navigate life’s transitions with dignity and grace? At Comfort Keepers, we’re looking for a Life Care Coordinator to join our team in a flexible, part-time capacity—bringing professional insight, emotional support, and thoughtful coordination to clients and their families throughout Northern Michigan.


This is more than a job—it’s a calling. You’ll serve as a trusted partner to clients facing complex needs, ensuring they receive personalized guidance and access to community, healthcare, and legal resources. You'll also support meaningful conversations around quality of life, legacy planning, and end-of-life care.


What You’ll Do

  • Provide compassionate, proactive support to seniors and their families as they navigate aging, caregiving, and healthcare planning
  • Coordinate essential life needs: from organizing mail to arranging appointments, transportation, and financial assistance (in collaboration with legal parties)
  • Collaborate with caregivers, physicians, and community partners to deliver truly holistic care
  • Conduct psychosocial assessments and maintain thoughtful documentation
  • Mediate sensitive conversations with empathy and clarity, supporting emotional well-being through transitions and losses
  • Encourage engagement in community events, life celebrations, and opportunities for joy

Who You Are

  • A licensed social worker (LBSW or LMSW), in good standing in the State of Michigan
  • Experienced in gerontology, end-of-life care, family systems, or social services related to older adults
  • An excellent communicator—skilled in difficult conversations, with the presence and professionalism to support families through emotional and practical decisions
  • Organized, adaptable, and comfortable working independently
  • Warm, reliable, and community-minded—with a true passion for advocacy and care
