Job Description:

Under the general supervision of the Mid-Cities Area Manager, the Client Care Coordinator is responsible for extending our reach in a new franchised area, building relationships, promoting our brand and helping achieve census & revenue objectives for our territories in NE Tarrant County and Denton County. This person will both build awareness AND manage accounts. CCC will oversee care and be the point of contact for families and clients as well as field calls, inquiries. In conjunction with the Sales Manager will help generate leads & referrals through networking within the senior community, i.e. with key physician groups, hospice agencies, and facilities (rehab, nursing); relationship building with other referral sources such as the VA, hospitals and clinics; regular interaction with senior living, including IL and AL. Referral sources include a broad array of different individuals: primary care doctors, nurses, physical therapists, social workers, case managers, discharge planners, but also bankers, trust/estate attorneys, wealth advisors, fin'l planners, and insurance brokers. Professional will be responsible for a dozen meetings per week, reporting actions & results on a regular basis, should be able to discuss successes and opportunities, and changes to the plan.

Min. Qualifications:

- Two-year associates degree program in Communications/General Business or Health Sciences. (License in healthcare preferred - LVN/LPN)

- Previous experience as a caregiver with certification: CNA, PCT, CMA or HHA 

- Five years of measurable success and progressively more responsibility in case management, with comfort level in marketing, PR, or business development preferable in the healthcare industry.

- At least one year of prof. experience in healthcare services

- Demonstrated ability to connect with others, communicate professionally and work independently

- Geographic tie to NW Tarrant county or Denton county, and existing relationships

Essential Functions:

  • Achieve revenue objectives for assigned territory with the primary goal of bringing in new business to increase census and overall market share. Target accounts include, but are not limited to: physician groups, home health & hospice agencies, assisted living facilities, skilled nursing, rehab, group homes, rehab centers, churches, senior centers, YMCA, etc.
  • Performs all phases of sales activities, including introductory calls, follow up calls & visits, analysis of client needs, explanation of services, intake calls, and follow-up as necessary with prospects
  • Establishes a networking plan and actively uses it as a working tool, which is revisited as requested.
  • Tracks all activity in a CRM system.
  • Participates in regular meetings among healthcare professionals as well as support groups and educational sessions
